Advanced Directives

Many people often have ideas about how they would be like to be looked after should they reach a point in their lives where they can no longer express themselves. These ideas can be written down and named an Advanced Directive. Often this is used as a way to help Clinicians maintain your personal dignity and keep your care personalised. If you decide this is right for you and would like us to hold a copy then it will become part of your record and can be referred to at a future date if needed. Advanced Directives are personal statements about how people wish to be treated. As such we cannot advise you of how to write them. It would also be important to keep it up to date should your needs change, and it can of course also be cancelled. If you would like us to hold an Advanced Directive for you then please speak to any of the staff or you can send it to us in any commonly used hard copy or electronic format with your personal instructions.
