Sick Notes

You do not require a doctor's sickness certificate for any illness lasting six days or less. Your employer may, however, require you to complete a self-certification form, which is available from your employer or from the gov.uk website. 

For any illness lasting longer than six days you will need to phone reception to request a sick note. Usually, if this is a recurrent illness or there is evidence in your medical record of the illness, for example A&E attendance, a message can be sent to the GP and a sick note will be issued. If there is no evidence in your medical record of your illness, you will be required to discuss this with a GP. Please note, that a request for a sick note is not urgent and will be booked as a routine call.

For any subsequent renewal of a certificate, usually this can be done via reception and a message to the GP, unless a review is needed, where a call will be booked for you.
